Person-centred care involves working with individuals to plan and provide care and support that meets their unique needs. It places the individual at the heart of care, enabling them to control how their care and support are delivered. This approach minimises the risk of negative, unfair or harmful treatment, including neglect. Person-centred planning, a common approach in social care, has four foundational principles: - The belief that individuals are capable of planning for themselves: Focus on their strengths and abilities. For example, supporting an individual who wishes to decide which mobility aids they prefer to assist in walking short distances rather than using a wheelchair. - Ownership of the care plan: Craft the care plan in the first person to signify that it's owned by the individual, for instance, 'I would like to try a walking frame when moving around the house and for short outdoor distances instead of using my wheelchair'. - Individual control over choices: Facilitate the individual to exercise control over their choices as much as possible. For example, support the individual in trying out the walking frame. - And finally, a better life through planning: The care plan aims to improve the individual's life rather than to fit them into an existing service. For example, find the best walking frame for them within the available resources, or support them in sourcing a suitable frame elsewhere if necessary. In health organisations, person-centred care is centred around compassion, dignity, and respect, which are crucial when involving people in their own care. Shared decision-making positions individuals as equal partners in their healthcare journey. Public involvement includes people in decisions about the design and delivery of services, such as involving communities in decisions about the services that will be provided. Promoting dignity is paramount. By valuing everyone, respecting their views, choices, and decisions, not making assumptions about their preferences, and working with compassion and person-centred values, you are upholding their dignity. This ethos lies at the heart of delivering person-centred care.
